Professor Patrick Hudson

'New thinking in safety: How do we achieve zero?' Tuesday 9 August, 9:00am

Patrick Hudson is one of the world's leading authorities on the human factor in the management of safety, in the oil and gas industry, in commercial aviation and in medicine. He was the Project Leader of the Tripod Research program for Shell International since 1987, was closely involved in developing Shell's approach to safety management systems after the Piper Alpha disaster and led Shell Group's Hearts and Minds program on the development of safety culture in the Oil and Gas industry. He is now leading a new program for Shell on high consequence low probability accidents. He also advises Total, British Gas, ENI-Agip and BP.Professor Patrick Hudson photo

Professor Hudson's involvement with aviation has included working with companies such as British Airways, Swissair, Cathay Pacific, Emirates, KLM, Malaysian and Singapore Airlines, Boeing and Airbus. He often advises the United Nations International Civil Aviation Organization (ICAO) based in Montreal and is regularly engaged by the Dutch, US, Singaporean, Australian and Canadian aviation regulatory authorities.

Professor Hudson is increasingly asked to advise other industries, including the steel and mining industries. In 2005 he was also appointed to the International Safety Expert Team overseeing the European Union's main nuclear research reactor. He was one of the authors of the report on patient safety in the Netherlands for the Minister of Health in 2005.

 
 

Tom Maher

Tom Maher was named Australia Region Vice President and Managing Director for Apache Energy Limited in December 2009. 

Prior to his Australian assignment, Tom spent 8 years working at Apache Egypt Companies in Cairo.  His position as Exploration Manager in Egypt saw Tom focus on exploration of new acreage acquired in government bid rounds, via farm-in or acquisition. Tom participated and later led a team that made a number of significant oil discoveries in the Abu Gharadig and Faghur Basins in the Western Desert of Egypt.  Subsequent and continued development of these discoveries has resulted in present day production of over 69,000 barrels of oil per day and 55 million cubic feet of gas per day.

Tom first joined the company when Apache Corporation acquired properties from Occidental in 1986.  He was located in the US Mid-Continent Region, Tulsa, Oklahoma commencing work as a geologist and moving on to Geoscience Manager and Exploration and Development Manager.

Tom holds a bachelor's degree in geology from the University of Massachusetts, a master's in Geology from Miami University (Ohio), and an MBA from the University of Phoenix. He is an AAPG Certified Petroleum Geologist.

 
 

SL Sreedhar

Sreedhar is a chemical engineer who started his career in India working for a contracting company involved in design and construction of steel, power and gas plants. He joined Santos twenty years ago and has held roles in mechanical and pipeline design, plant operations, plant integrity, project development and risk management. This all round experience puts him in good stead to carry out his role as Principal Engineer, Process Safety Management over the last five years.

 
 

Dr Melinda Hodkiewicz

Associate Professor Melinda Hodkiewicz is the head of the Engineering Asset Management (EAM) Program at the University of Western Australia, Chair of the Standards Australia committee and member of the ISO committee responsible for development of an ISO Standard for asset management. She is responsible for UWA's postgraduate Business & Engineering Asset Management and Executive courses and works with a number of global organisations on professional development for Asset Managers. She collaborates regularly with infrastructure, local government and the resource sector on multi-disciplinary research projects particularly in the areas of prognostics of asset failure, asset data quality and life cycle costing.

 
 

Mhairi Glover

Mhairi Glover is the Senior Development HSS Manager for Woodside where she is responsible for facilitating and guiding the Development Function in the effective implementation and monitoring of Health, Safety and Technical Integrity Management and operating Standards.  Mhairi has almost 20 years' experience in technical safety across the oil and gas and mining industries.  After completing a Masters in Operational Research at Strathclyde University she started working as a safety/risk consultant in Aberdeen working on safety cases for drilling facilities and researching best available technologies for evacuation, escape and rescue from offshore facilities. Mhairi has been in Australia for the past 15 years working first in consultancy before joining Woodside 10 years ago. In Woodside Mhairi has been involved across many onshore and offshore projects driving best practice safety in design processes and facilitating improvements on how risk based decision making is made. 

 
 

Steve Pearson

Scott Cornish

Scott Cornish is Acting Health, Safety and Security Manager for Origin's upstream business and is responsible for delivering both Origins' Emergency Management systems and for providing guidance and advice on Health and Safety across Origin's upstream businesses throughout Australia.

With a career spanning over 30 years in the oil and gas industry, Scott has held a wide variety of roles including maintenance and operations, major projects from FEED through to commissioning, shut downs and health and safety.

Scott has also built a career in Crisis Management being actively involved within the Australasian Region.  His experience ranges from managing refinery incidents, tank farm fires, and hazardous material incidents as well as being a key role within Origin's Queensland flood Emergency Management Team for both regional sites as well as the Head Office in Brisbane.

 
 

John Sargaison

John Sargaison is the Manager Health, Safety and Security with Santos Queensland.  He has 34 years' experience in health and safety with 23 of those years in the mining sector and the last 11 in the oil and gas industry with Santos. He was the mining industry health and safety representative that participated in developing the current Queensland coal and metalliferous mining legislation and stewarded the establishment of the privatised Queensland Mines Recue Service. 

Since commencing work in the oil and gas industry with Santos, John has developed and overseen the implementation of the Santos EHS Management System and a number of supporting systems and tools such as the Significant Hazard Risk Register.  For the last two years he has headed up the Health, Safety and Security team for Santos GLNG.  

John has a Graduate Diploma in Occupational Health and Safety from Queensland University of Technology and a MBA from University of Southern Queensland.

 
 

Greg Harrison

Sarah Harrison

Dr Barry Chesson

He holds a PhD in chemical safety, an Honours Degree in Physical Chemistry, a Graduate Diploma in Natural Resources and a Masters Degree in Applied Science (Health Science).  He is a Fellow of the Australian Institute of Occupational Hygienists and is a Certified Occupational Hygienist with that body.  He is currently a member of the Executive of the AIOH and in 2012 will serve a one-year term as President of the national body.  He holds an appointment as Adjunct Associate Professor at Curtin University's School of Public Health. 

Barry has had a long involvement with government, industry and unions in the development of OHS policy, standards and regulations for Western Australia and is currently serving a further term as an Expert Member of the WorkSafe Commission of WA.

 
 

Jim Winter

Namek Jivan

Chris Jackson is the Safety Manager with Apache Energy.  He has been in Safety Management positions with oil and gas companies since 1993. Prior to this Chris had a variety of jobs including roughneck, production operator, teacher, safety advisor offshore and Risk Management consultant and manager across industry and the government sector.

Chris was involved in the ACE initiative in 2000 that first looked at the issue of a common induction for the oil and gas industry and through Apache has been actively involved in the Common Safety Training Program (CSTP).

 
 

Peter Wilkinson

Peter Wilkinson is Principal Consultant with Noetic Risk Solutions specialising in the oil and gas and mining industriePeter Wilkinson photos. In the last 18 months Peter has completed assignments for clients in Australia, China, UK, Malaysia, Timor Leste, New Zealand, the US Gulf of Mexico as well as for the Australian Government on the Montara oilfield blowout.  

Peter has an unusual mix of regulatory and industry experience. From 2005 to 2009 Peter was the Group Manager for Operational Excellence and Risk in Caltex Australia and a member of the Caltex Leadership Team. He oversaw the building of a process safety culture and the introduction of an enterprise wide risk management framework rated by McKinsey and Co as an example of excellence.

From 2001 – 2005, Peter was the principal "architect" for the development of the National Offshore Petroleum Safety Authority. During this period, Peter was also an advisor to Neville Wran during his review of mine safety in New South Wales, and with Andrew Hopkins advised the West Australian Government in their review of mine safety regulation.

From 1991 - 2001 Peter worked in the North Sea for the UK's Health and Safety Executive, Offshore Safety Division regulating the offshore petroleum industry following the Piper Alpha disaster. This included work on developing the safety case regime.

 
 

Pat O’Bryan

Bruce Lake

Bruce Lake is the Managing Director of Vermilion Oil and Gas Australia which owns and operates the Wandoo production facility and field offshore Dampier.

Bruce has been an APPEA Board member since 2008 and has chaired the APPEA Health Safety and Operations committee since that time, having been an active member of the committee for much of the previous ten years. He is also the Chair of the APPEA/Industry Taskforce response to the Montara and Macondo incidents.

Bruce has over 30 years of experience in the oil and gas industry in Australia and New Zealand in Engineering, Operational and Management roles. He has worked both onshore and offshore for a broad range of operating companies including majors and independents.
